Meet the Board Quality Committee 2017-2018

MARION THOMSON HOWELL (Quality Committee Chair)
Marion is an entrepreneur, nationally recognized learning consultant and active community contributor. She began her career as a lecturer in Health Education at the University of Windsor, later moving into the corporate world. Marion is now President of Shaughnessy Howell Inc., an award-winning training organization. Marion has been an active volunteer in the K-W community with the Canadian Cancer Society and school associations and has been a member of the boards of the Waterloo Catholic District School Board, St. Jerome’s University and now St. Mary’s General Hospital. She lives in Kitchener with her husband, Gary. 

LINDA FABI
Linda is the retired Director of Education for the Waterloo Region District School Board (WRDSB) where she served public education for 37 years. In retirement Linda sits on a number of non-profit arts and culture and education boards in the Region, and more recently, St. Mary’s General Hospital Board where she served on the Mission and Governance Committee for 2 years, and is currently a member of the Quality Committee. Linda has also been appointed to the Minister of Education’s Curriculum Advisory Council and is Chair of the International Women’s Forum (IWF), Waterloo Chapter.  Linda earned a B.A and an M.A. at Wilfrid Laurier University where she was named one of “Laurier’s Top 100 Alumni of Achievement in 100 Years” in addition to other awards and recognition including most recently, K-W Oktoberfest’s “Woman of the Year – Lifetime Achievement Award”.  Linda resides in Cambridge.

AMY HAZELWOOD
Amy is the Communications and Marketing Director for the St. Mary’s General Hospital Foundation. Following a 20 year career in the consumer branded products industry, Amy found herself yearning for an opportunity to dedicate her extensive background in marketing to a cause more worthy.  Having joined the Foundation in June of 2015, Amy is responsible for the communications planning, promotion and advertising of all events and fundraising initiatives.  In her free time Amy can most often be found making memories with her partner Simon and their two daughters here in Kitchener.

AMOL KARNICK
Amol is President and CEO of Kitchener based start-up called KA Imaging. A seasoned executive in the medical industry, Amol has over 20 years worth of experience with medical devices with a focus in medical imaging space. With a Bachelor’s degree in Engineering from the University of Waterloo and a Master’s degree in Electrical Engineering from McGill University, Amol has managed multidisciplinary teams, including sales, engineering and operations.

DIETER KAYS (Past Chair, Board of Trustees)
Dieter Kays is the recently retired CEO/President of FaithLife Financial. A Rotarian, Dieter currently is on boards for the Ombuds Service for the Canadian Life and Health Insurance Industry, Lutheran Church-Canada Worker Benefit Board and St. Mary’s. He has a PhD in organizational leadership from Wilfrid Laurier University, a Master of Divinity degree from Concordia Seminary, St. Louis, Missouri, and a Master of Social Work from Washington University. Prior to entering the financial services industry, he was CEO/President of Lutherwood in Waterloo. Dieter has done post graduate work at Stanford University’s business school, the Wharton School of Business, and Rotman, University of Toronto.

CHRIS McINTYRE
Chris is the Director of Product Management, Healthcare for Calgary Scientific. He has previously held several senior product positions in medical imaging and mobile health software, including Agfa Healthcare and Unitron Hearing, and has run his own human factors consulting business. Chris also spent 14 months volunteering in Sri Lanka as a strategy advisor to assist in the rebuilding efforts following the 2004 tsunami. Prior to joining the St. Mary’s Quality Committee in 2017 as a community representative, he served on the Resource Planning and Utilization Committee since 2014. Chris lives in Waterloo and is married with two children.

TOM MOTZ (Chair, Board of Trustees)
Tom is Chair of the board of St. Mary’s. He has owned and been involved in a number of industries including newspaper publishing and precision manufacturing and is a member of the Golden Triangle Angel Network. Tom has a Bachelor of Business Administration degree from Wilfrid Laurier University. Tom has served on the boards of the Catholic Family Counselling Centre and Wilfrid Laurier University and currently serves on their Counsel of Advisors. Tom is a member of the Kitchener-Conestoga Rotary Club. He served on several committees as a community representative before joining the board in 2007. Tom is married with two sons.

TERRY O’SULLIVAN
Terry is an innovative and versatile strategist and a local business owner. With decades of experience he offers a rich mix of leadership, operations, risk management, sales and business development. Terry and is wife Jennifer were both born and raised in Kitchener-Waterloo. He attended Wilfrid Laurier University for a double major honours degree in Political Science and Business. Terry has lived in Waterloo Region for 50 years and has volunteered with numerous charities, professional memberships and has served on many boards. Currently he sits on the board with Mag-Tech Renewable Energy Corporation as well as volunteering with St Mary’s Quality Committee.

LOIS PETERSON
Lois has 33 years’ experience in education with WCDSB, and, is currently on WLU, Faculty of Education. She is the Co-Chair of the Patient and Family Advisory Committee for St. Mary’s Hospital. Lois has served on numerous boards in the community and is a life-long resident of the Waterloo Region.

WARREN STAUCH
Warren was born in Kitchener and grew up near St. Mary’s Hospital. He has B.A. and M.A. degrees in Geography from Waterloo Lutheran University (now Wilfrid Laurier University). Warren retired in 1999 after 30 years as a teacher and department head at Galt Collegiate and Elmira District in addition to teaching twice at Eastwood Collegiate. In retirement, Warren is heavily involved in many heritage groups and service clubs. Currently, he is co-president of the St. Mary’s General Hospital Volunteer Association and Director of Businesses (Tim Hortons, Gift Shop and Lottery). He and his wife, Martha, enjoy travelling the world. For 50 years, Warren has acted as a step-on guide taking visitors and locals on shunpiking day-trips within the Region and throughout the Grand River watershed.

LISA TERLEVIC, MRT(R), RT(R), AC(R), BAppSc(Medical Imaging)
Lisa Terlevic started her St. Mary’s General Hospital career as a Registered Medical Radiation Technologist (MRT(R)) in 1990. Experienced in diagnostic imaging procedures, including general, mammography and CT, she was the Clinical Coordinator for Fanshawe College Medical Radiation Technology students from 1995 – 2003. She was a member of the planning and design team for the Cardiac Catheterization Labs, and continued on as the Lead MRT since the opening of the labs in 2001. She obtained her Advanced Certification in Radiography in 1995, and her Bachelor of Applied Sciences (Medical Imaging) in 2004. Lisa has recently accepted a temporary position as Operational Supervisor for the Heart Rhythm Program, and is excited to be part of the development and implementation of the Electrophysiological (EP) suite. She lives in Kitchener with her husband and three energetic girls.
