Nuclear Medicine & Diagnostic Imaging: for referring physicians

The System Coordinated Access Program (SCA), in partnership with the Regional Diagnostic Imaging Integrated Program Council, is inviting medical specialists in your department to send electronic referrals and/or receive electronic referrals. Specifically, the Council is implementing eReferral for all hospital outpatient diagnostic imaging requisitions by early 2018. This development will allow health care providers to send electronic referrals to any hospital DI site in Waterloo Wellington using a common requisition form.

Click here for a list of electronic requisition forms available under the Ocean Health Map heading.
