Patient records, release of information and privacy

At St. Mary’s General Hospital we are committed to protecting the privacy and confidentiality of patient health information. Our policies and procedures comply with provincial and federal legislation.

The patient health record is the property of the hospital, however, the information is accessible to you. Health information is retained for the period required by law. If you were an adult (18 years according to the legislation) at the time of treatment, the information is retained ten (10) years past the date of treatment. In the case of a child under the age of 18 years, the information will be retained for a period of twenty-eight (28) years. Radiology films are retained for five years. Films are not stored on site.

If you are a patient who would like to access your health information, contact your family doctor or our Patient Records (Release of Information) office. As of May 1, 2017, the office is open Monday to Friday, from 8:00 a.m. to 4:00 p.m. The office is located in the basement of the Gordon A. Mackay building, adjacent to the main hospital on Queen’s Blvd. The building has an entrance off Queen’s Boulevard, or you can access it through the Main Lobby of the hospital. A volunteer will be pleased to assist you.

Non-medical care providers are granted access to patient information only when the patient has given authorization by signing a consent form. There is an administrative cost for non-medical requests.
