Medical house calls part of geriatric pilot project

A nurse practitioner, specially trained in geriatrics, will visit frail elderly patients in their home, accompanied by a geriatrician if more expertise is needed. The goal of the project, which also includes a monthly wellness clinic at a Waterloo retirement home, is addressing the growing number of older adults admitted to hospital for chronic disease management, acute illness and social issues that could be handled safely in the community.
