Nuclear Medicine & Diagnostic Imaging: for referring physicians

The Diagnostic Imaging Integrated Program Council, in partnership with the System Coordinated Access Program (SCA) have developed standardized regional diagnostic imaging outpatient requisitions. This means that all hospitals within Waterloo Wellington Region are now accepting the same forms for outpatient DI requisitions (i.e. for MRI, CT, etc.).

The standardized regional forms can be sent by fax, electronically (i.e. EMR custom form) or by an electronic referral solution (eReferral).

Please convey to your patients that they may receive a phone call or voice mail from Nuclear Medicine relating to their appointment

As of February 6, 2023, all primary care providers (PCPs) within Kitchener-Waterloo can make referrals through SCOPE (Seamless Care Optimizing the Patient Experience).

Through a single point of access, primary care providers are connected with a Nurse Navigator who will assist them with navigating the health care system, both the hospital and community settings, to ensure providers and patients are connected to the appropriate resources in the most timely way possible.

PCPs will enter a SCOPE eReferral via Ocean which is reviewed by the SCOPE Nurse Navigator who will then reach out to the services needed.
