Specialized Geriatric Medicine

The Inpatient Specialized Geriatric Consultation Service conducts a comprehensive assessment and provides recommendations for older patients at St. Mary’s General Hospital who have complex medical and functional issues. The team is composed of a Geriatrician and an Advanced Practice Nurse, and is supported by Registered Nurses, Occupational Therapists, Physiotherapists and Social Workers. Referrals are welcomed from all inpatient teams at St. Mary’s.

Geriatric Medically Complex Clinic (GMCC)

In March, the Geriatric Medically Complex Clinic (GMCC) transition backed to St. Mary’s (911 Queen St. S., Kitchener). Patients accessing the GMCC now receive care at the hospital. 

Referrals:

There is no change to how patients are referred by physicians to the GMCC. Please continue to make referrals using the Waterloo Wellington Specialized Geriatric Services Clinical Intake Form faxed to: 1-888-205-1491 and remind your patients that the clinic has moved back to St. Mary’s.

Patients Visiting the Clinic:

Patients can bring a Care Partner essential to their wellbeing to their appointment, provided that this person is fully vaccinated. This is in keeping with St. Mary’s mandatory vaccination policy for Care Partners. Paid parking is available for patients and Care Partners directly across from the hospital in the Visitors’ Lot.

Services such as pharmacy and lab testing are available onsite at St. Mary’s and our team of staff and Nurse Practitioners are available to provide support, answer questions and work with the patient’s Care Partner for informed and collaborative care.
